is accurate, truthful, and complete to the best of my knowledge and belief. I understand and acknowledge that any
false, misleading, or incomplete information may result in disqualification from the vendor selection process,
termination of any existing contracts, and/or legal consequences as deemed appropriate under applicable laws and regulations.
I further acknowledge that PSI Engineering reserves the right to verify all information provided and may request
additional documentation or clarification at any time. I hereby authorize PSI Engineering to conduct background checks
and contact the references provided herein. I commit to promptly notifying PSI Engineering of any changes
to the information provided within ten (10) business days of such changes occurring.
